An independent research institute evaluated the clinical effectiveness and value of oveporexton, an investigational orexin receptor 2 agonist for narcolepsy type 1.
Key takeaways:
The Institute for Clinical and Economic Review (ICER) recently released a final evidence report assessing the comparative clinical effectiveness and value of oveporexton for narcolepsy type 1 (NT1).
Developed by Takeda, oveporexton is a first-in-class oral orexin receptor 2 agonist. ICER’s report was published on June 8, 2026; Orzeyful (oveporexton) has since received US Food and Drug Administration approval (in August 2026).
“The narcolepsy treatment landscape includes several pricey medications that do not provide complete relief for patients,” says Foluso Agboola, MBBS, MPH, senior vice president of research at ICER, in a release. “ICER’s analyses found that oveporexton, the first medication for narcolepsy type 1 that directly addresses the underlying cause of the condition, is effective in promoting wakefulness, improving quality of life, and appears to offer better health benefits than current options. The manufacturer choosing a value-based price for oveporexton would help de-escalate the cycle of high prices being met with onerous access challenges. Our independent analysis can offer a roadmap for improving affordability and access for patients and the health system.”
Based on its review of clinical trials, ICER judged oveporexton to have a B+ evidence rating compared with no pharmacological treatment, indicating moderate certainty of a small or substantial net health benefit. When compared to active treatments like modafinil or armodafinil with venlafaxine, sodium oxybates, and pitolisant, oveporexton received a C++ rating, indicating moderate certainty of a comparable, small, or substantial net health benefit.
The manufacturer has not announced a US price. ICER calculated a health benefit price benchmark of $50,400 to $59,400 per year. The report notes that if priced within this range, all potentially eligible patients could be treated without triggering a potential budget impact threshold.
Following a voting session, a policy roundtable of clinical experts, patients, payers, and the manufacturer convened to discuss pricing implications and fair access. Key recommendations stemming from the report include:
