To the Editor Rozé and colleagues report that prophylactic intravenous acetaminophen initiated within 12 hours after birth increased ductal closure by day 7 and reduced subsequent rescue therapy, yet did not improve the primary composite outcome of survival without major neonatal morbidity at 36 weeks’ postmenstrual age. This dissociation between an anatomically successful surrogate end point and unchanged clinically meaningful outcomes deserves emphasis because it challenges the long-standing assumption that earlier ductal closure is inherently beneficial in extremely preterm infants.
