Prasugrel—The Default P2Y12 Inhibitor After PCI for ACS?

Dual antiplatelet therapy remains a mainstay in patients undergoing percutaneous coronary intervention (PCI) after an acute coronary syndrome (ACS), yet selecting the most appropriate P2Y12 inhibitor remains quite challenging. JAMA Cardiology features a landmark network meta-analysis by Maqsood and colleagues using aggregate data from randomized clinical trials comparing clopidogrel, prasugrel, and ticagrelor in patients undergoing PCI, mostly with ACS. This timely and well-conducted exercise in evidence synthesis, despite the limitations inherent to any similar scholarly effort, provides important insights for clinical practice.
