The process had been thorough. The references were strong. The candidate appeared to be the right person for the job. Yet it took only six months for Carola Lemne and the rest of the board to realise that something was not working.

The newly appointed chief executive struggled to make decisions and kept information too close to themselves. These were qualities that had not emerged during the recruitment process.
